PLEASE READ THE FOLLOWING INFORMATION CAREFULLY BEFORE COMPLETING THE APPLICATION FORM:

 

1.     Complete the application form on BOTH sides.  Please print in BLOCK LETTERS.

 

2.     Submit only ONE application form for each level.

 

3.     When making your course selection(s) please check the Course Schedules for the correct course code and avoid timetable clashes.

 

4.    Timetable for and Location for Enrolment in all Certificate Courses

Date:     Sept 16 – 19, 2008

Time:     10:30 am – 4:00 pm

Venue:   Outside the Main Library (covered area)

 

For enrolments after Sept 19, 2008, please approach the School Office, Room 706, K.K. Leung Building.

 

Admission is on first-come-first-served basis.

         5.     Failure to observe the above procedures will render your application void.

APPLICATION RESULTS AND TUITION FEES:

 

After submission of application form to the School General Office, you will be informed immediately if you will be allocated a place in the courses.  If you are offered a place, please obtain a Payment Instruction sheet to pay the tuition fee specified for the course you are admitted to before the deadline specified on the payment instruction sheet.  The tuition fee paid is non-refundable. The offer of admission will be nullified and the place will be given to another student if the carbon copy of the pay-in slip or the ATM generated payment advice is not received on the deadline specified.

LEAVE OF ABSENCE:

 

A student who cannot attend classes because of medical reasons should write to the Head of School not later than two days after the leave of absence.  Sick leave should be supported by a certificate signed by a registered medical practitioner.

 

A student who desires leave of absence for non-medical reasons should apply to the Head of School in writing prior to the commencement of leave, stating the reasons for which the leave of absence is sought and provide supporting documents.

 

 

 

       
 

WITHDRAWAL:

 

If for any reason you cannot attend the course for which you have registered, you should notify the Head of School in writing, so that your place can be offered to another applicant in time
